There are several different ways to pay for services. Cash, FSA/HSA or I am in-network with most major insurance plans, including Anthem Blue Cross, Aetna, United Healthcare, and Medicare. Depending on your coverage, your visits may be fully or partially covered.

If you would like to use insurance for your visit, the first step is to schedule an appointment using the link below. After that, we will review your insurance information, verify your benefits, and confirm your appointment once everything is in place.

Send Your Insurance Information
After scheduling, email a copy of the front and back of your insurance card along with your ID to healthwins.insurance@gmail.com
We Verify and Confirm
Once we receive your information, we will verify your benefits and request a referral from your provider if needed. After your coverage is confirmed, we will finalize and confirm your appointment.
